의료자원의 임상적 성과를 파악할 수 있는 방법을 제안하기 위하여 본 연구는 확률변경분석과 맘퀴스트 생산성 지수를 통해 지역별 의료자원 투입이 연령 표준화 사망률에 미치는 영향을 분석하였다. 지금까지 보건 의료 부문 효율성 관련 선행연구가 의료자원을 활용하는 기관의 운영 효율성에 초점을 두었다면, 본 연구는 거시적 측면에서의 임상적 성과 효율성에 초점을 두었다. 의료자원을 효과적으로 투입하여 건강 수준을 향상시키는 것은 일종의 성과 측면의 효율성으로 이해할 수 있으며, 이는 보건 의료 부문의 재정적 지속가능성을 효과 측면에서 파악할 수 있다는 데 그 의미를 찾을 수 있다. 이에 2006~2013년, 우리나라 16개 시도를 분석의 범위로 하고, 의사 수, 전문의 수, 병상 수, 수술 수와 연령 표준화 사망률을 투입하고, 확률변경분석과 맘퀴스트 생산성 지수 분석을 수행하였다. 이때, 횡단면, 시계열 자료가 복합되어 있는 자료의 구조를 고려하기 위해 패널확률변경분석 및 맘퀴스트 생산성 지수를 각각 적용하였다. 두 분석모형 모두 의료자원의 기술적 효율성이 산출물 생산에 큰 영향을 미치는 것으로 나타났다. 본 연구에서 시도한 확률변경분석과 맘퀴스트 생산성 지수를 사용하여 지역별 의료자원의 성과 측면을 고찰하는 것은 의료자원의 최종적 목표인 임상적 측면에서의 건강 성과를 효율적으로 판단한다는 데 그 특징이 있다. 분석결과를 바탕으로 각 지역별 의료 자원의 성과적 효율성이 어떻게 나타나고 있는지 파악할 수 있으며, 공급 측면의 비용 절감을 위한 정책 방향을 제시할 수 있다는 점에서 본 연구의 의의가 있다고 할 것이다.;In order to propose a method to measure the performance of medical resources, this study empirically analyzed the impact of regional medical resource input on age standardized mortality rate through probability frontier analysis. If prior studies so far focused on the operational efficiency of institutions that use medical resources, this study focused on performance efficiency in the macro perspective. As is already well known, the probability frontier method measures the inefficiency of the analysis object in consideration of the error variation, overcoming the disadvantages of the existing data envelope analysis method. In our empirical analysis, we examined16 metropolitan cities and provinces in Korea for the period 2006-2013, and employed the panel stochastic frontier method and Malmquist Productivity Index Analysis by putting the number of doctors, the number of specialists, the number of beds, the number of operations and the age standardized mortality rate. The result showed that all the medical resources allocated to each region decreased the age-standardized mortality rate. Regional inequality in medical resources distribution should be dealt with in depth from various perspectives. In this respect, the method of applying the stochastic frontier analysis to measure the performance of regional medical resources has the advantage that the inefficiency of each input element can be examined more in depth, and this study is meaningful in this respect.
본 연구는 노인요양시설 요양보호사의 조직 내 갈등이 이직의도에 미치는 영향과 이 변수들 간의 관계에서 사회적 지지의 조절효과를 실증적으로 검증함으로써, 요양보호사들의 갈등을 효율적으로 관리하고 조직성과를 향상시키는 방안을 모색하는 데 목적이 있다. 이러한 연구목적을 달성하기 위해 경북지역 10개 시･군 소재 22개의 노인요양시설에 종사하고 있는 요양보호사를 대상으로 설문조사를 실시하여 자료를 수집하였다. 수집된 자료는 SPSS WIN v21.0과 AMOS v18.0을 활용하여 기술통계, 상관관계 분석, 구조모델 분석을 실시하였다. 본 연구의 결과는 다음과 같다. 첫째, 노인요양시설 요양보호사의 조직 내 갈등은 이직의도에 정(+)의 영향을 미치는 것으로 나타났다. 둘째, 조직 내 갈등과 이직의도 사이에 사회적 지지의 조절효과가 작용하고 있으나, 그 효과는 사회적 지지 유형에 따라 다르게 작용하고 있었다. 셋째, 조직 내 갈등의 하위요인들과 사회적 지지 유형에 따라서 조절효과의 작용이 상이하게 나타나고 있었다. 이 연구는 노인요양시설 요양보호사의 조직 내 갈등요인이 요양보호사의 이직의도에 미치는 영향과 사회적 지지의 조절효과를 분석함으로써, 노인요양시설의 효율적이고 효과적인 관리에 유용한 실천적 및 정책적 함의를 제공했다는데 의의가 있다.;This study aims to find ways to efficiently manage organizational conflicts of care workers working in the elderly care facilities and further improve the organizational performance by empirically verifying the influence of the organizational conflicts on their turnover intention and the moderating effect of social support between these variables. For these purposes, we collected data by conducting surveys among the care workers engaged in the 22 elderly care facilities in 10 cities and counties of Gyeongbuk province. The data collected were analyzed through the Descriptive Statistics, Correlation Analysis, and Structural Model Analysis with the SPSS WIN v21.0 and the AMOS v18.0. The results are as follows. First, organizational conflict of care workers was found to have a positive effect on their turnover intention. Second, although a moderating effect of social support was found between organizational conflict and their turnover intention, the effect varied depending on the types of social support. Third, the moderating effect varied depending on the types of conflict and social support. This research provides theoretical, practical and policy implications for the efficient and effective management of the Elderly Care Facilities by analyzing the influence of organizational conflicts among care workers on their turnover intention and the moderating effect of social support.
