본 연구는 주관적 건강 상태에 영향을 미치는 지역 특성을 경험적으로 파악하고자 하였다. 이는 개인의 주관적 건강 상태를 고양시킬 수 있는 기반 조성을 위해 지역적, 미시적 차원에서의 도시계획 정책이 이루어져야 함을 역설하기 위함이다. 아울러 생태학적 이론을 바탕으로 개인과 지역사회가 위계 관계를 구성하고 있음을 고려하여 기존 연구와의 차별성을 도모하고자 하였다. 이에 따라 실증분석 방법론으로 다층순서형로지스틱모형(Ordered Logistic Multilevel Model)을 적용하고, 1수준 개인 변수에는 2013 서울서베이 자료를, 2수준 지역 변수로는 2013년도 서울통계(stat.seoul.go.kr)를 활용하여 각각 투입하였다. 지역 변수를 중심으로 분석결과를 간략히 요약하면, 1인당 도시공원 면적과 녹지면적, 가로녹시율와 같은 자연환경 중심의 쾌적한 환경(amenity) 요인이 주관적 건강 행복지수에 정(+)의 영향을 미치는 것으로 파악되었다. 지속가능한 개발(sustainable development) 기조를 바탕으로 할 때, 개인의 주관적 건강 행복지수를 고양시키기 위한 방안으로서 다른 부문과의 정합성을 유지하여 쾌적한 근린 환경 조성을 고려하는 것이 요구된다. 특히 현재 수립, 시행되고 있는 2030 서울도시기본계획(2030 서울플랜)과 같은 도시기본계획은 본 연구결과를 바탕으로 지속가능한 발전과 주관적 건강 행복지수를 동시에 고려할 수 있도록 부분적으로 도시공원과 녹지, 가로수 등 자연환경 중심의 쾌적한 환경을 효율적으로 배치할 수 있는 세부 계획을 미시적 차원에서 추가적으로 수립, 시행하여야 할 것이다.;The implementation of urban planning policy should come about at the regional level with ecological perspectives based on the social need for promoting individuals’ subjective health and wellbeing. Recognizing the hierarchical relationship between the individual and the community, this study examined the regional factors that affect people’s subjective health and wellbeing. For our empirical analysis we used Ordered Logistic Multilevel model. The data used for this study are from the 2013 Seoul Survey (for the individual) and Seoul Statistics (for the regional). The results indicated that environmental amenities such as local parks, green spaces and green streetscape had a positive association with local residents’ subjective health and wellbeing. Therefore, the effect of environmental amenities are empirically supported, which further argues for the importance of developing local amenities in promoting the wellbeing of local residents. Consequently, planning policies such as 2030 Seoul Plan should consider the regional characteristics and their potential effect on the wellbeing and health of Seoul citizens.
2012년 “공공보건의료에 관한 법률”의 개정으로 민간의료기관도 공공보건의료 기능을 수행할 수 있게 됨에 따라, 공공보건의료의 정의가 국공립 소유의 관점에서 기능 관점으로 재정의 되었다. 이는 민간조직의 공익성을 강화함으로써 공공의료의 총량을 늘리겠다는 취지로 해석된다. 그러나 그간 의료공공성에 대한 논의는 사회가 추구하는 가치, 혹은 결과로서의 의료 공공성에 집중되어, 이를 수행하는 조직의 공익성에 대해서는 합의된 틀이 부재한 상황이다. 본 연구에서는 의료기관의 공익성을 측정가능성 측면에서 이론적으로 검토하고, 의료기관의 공익성을 투입-과정-산출이라는 세 요소에 대해 살펴보는 통합 분석틀을 제시하였다. 또한 이에 근거해 우리나라 일부 민간의료기관의 공익성 인식과 평가, 의료기관의 조직운영, 의료서비스 제공의 공익성 현황을 조사, 분석하였다. 조사 결과, 민간의료기관들은 공공성을 수익성과 대립되는 가치, 정부의 통제로 인식하고 있었고, 주로 조직의 생존과 사회적 순응을 위해 공익 활동을 수행하고 있었다. 병원의 책무활동, 의료서비스 제공에서의 공공성, 조직운영과정에서의 민주성, 개방성 역시 제한된 수준으로 나타났다. 이러한 결과는 아직까지 우리나라 민간의료기관들의 공익적 역할 수행이 공공성의 가치를 보다 적극적으로 인식하고 추구하는 데까지는 이르지 못했음을 반영한다.;As of 2012, the Revised Public Health Care Law redefined public health care as function of organization from public ownership perspective. This aimed to increase private organization’s public activities to meet the expectation of policy goal to raise the overall publicness in Korean health care. However, there is no analytical framework on definition and measurement of organizational publicness even though there has been a large body of discussions around the publicness as a social value or service outcomes. We reviewed definition and measurement of the publicness of healthcare organizations and developed an analytical framework for evaluating the publicness in private organizations. Based on the framework we developed, we collected the survey data from twenty-eight private healthcare organizations and described the current status and perception on the publicness. According to the survey, the private organizations tend to perceive the publicness as follows: the competing value with profitability, or government’s control. Second, the private organizations performed the public activities mainly to make profit to run hospital and to promote social gain. Third, in the perspective of organization’s accountability and health services provision, it shows low publicness and limited democracy and transparency in organizational management process. These results show that the public activities of private healthcare organizations in Korea are mainly performed as an instrumental role to meet social demands.
이 연구는 남북한 사회보장제도 통합의 환경과 정책선택 등 현실 제약요인의 검토를 통해 한반도에도 독일과 같은 형태의 사회보장제도 통합이 가능할 것인지를 분석하였다. 통일당시 동서독과 현재 남북한 대치국면에서의 국제환경, 신뢰구축 정도, 경제력 차이로 판단할 때 한반도 통일이 독일식으로 전개될 가능성이 낮으며, 남북한 사회보장제도 통합이 독일식으로 급진전 되는 것도 바람직하지 않다. 우리는 북한의 평등급여와 남한의 차등급여, 남한 ｢사회보장기본법｣의 대북한 적용문제, 국가‧기업‧개인의 복지책임 문제 등 사회보장제도의 이질성을 동질적으로 수렴시켜야 하는 과제를 안고 있다. 남한주도의 사회보장제도통합 비용은 현재 남한 조세수입 규모의 최소 12%에서 최대 50%에 이를 것으로 추정되었다. 따라서 제도의 차이 및 재정제약, 급속한 통합 이후 예상되는 통일한국 경제의 성장잠재력 저하 등 현실적 한계를 고려할 때 사회보장제도의 통합은 점진적 개혁과 한시적인 차등 적용의 과정을 통해 완성하는 것이 바람직하다. 통일 시 예상되는 북한지역 주민의 남하이주에 대해 물리적 이주는 비현실적이며, 일자리 마련과 사회보장제도를 통한 자발적 잔류유인을 제공하는 적극적인 정책이 수립되어야 한다.;This study investigates the possibility of a German way of social security integration in the Korean peninsula. Unlike German reunification where the Western system of social security and welfare replaced that of the East within a short period of time after the unification, it is much less obvious that the South Korea is playing a dominant role for North Korea. Considering our estimation of the capital costs of social security integration under various circumstances and assumptions, a hasty decision about the full-scale integration of social security system prior to North’s taking matured market economic principles is far from desirable. A rapid integration may cause delay in economic and social union. We proposed that short-term social security support for the protection of basic livelihood security of the North Korean immediately after reunification is required. Emergency welfare assistance system including minimum living expense guaranteed and health protection for the people from the North right after the reunification should be promoted. Also, to prevent disorder caused by the probable exodus from the North to the South predicted with the reunification, it is very important to prepare a policy that induces North Koreans to voluntarily remain and work in the North.
