본 연구의 목적은 OECD 국가들 가운데 외부로부터의 압력에 의해 연금개혁을 실시한 그리스의 사례를 분석하고 우리나라 노후소득보장체계의 발전을 위한 시사점을 도출하는데 있다. 이를 위해 그리스 연금개혁의 경제적 맥락과 정치적 과정을 살펴보고, 연금개혁의 과정 및 연금제도의 내용을 분석하며, 연금개혁의 과제 및 방향을 전망하였다. 연구결과는 다음과 같다. 첫째, 2008년 경제위기 이후 EU·IMF로부터 구제금융을 받는 조건으로 강제적으로 이루어진 그리스의 2010년 연금개혁은 재정안정화에 초점을 맞추어 급진적으로 추진되었으며, 개혁의 강도는 그리스에서 이루어진 이전의 연금개혁은 물론, 다른 서구선진국들의 연금개혁과 다르게 상당히 큰 수준이었다. 그리스의 연금개혁은 외부의 압력에 의해 한 국가의 연금체계가 급진적이며 구조적으로 개혁될 수 있으며, 사회 구성원들의 요구가 충분히 반영되지 못하는 개혁이 될 수 있다는 것을 보여준다. 둘째, 그리스의 소득비례연금의 급여수준이 대폭 하락하여 총 소득대체율이 90%에서 60% 정도로 낮아졌으나 이에 대한 보완책이 미흡하여 앞으로 노인빈곤 문제가 더욱 심각해질 개연성이 있다. 그리스 사례를 통해 본 연구는 노인빈곤의 문제를 해결하기 위해 우리나라 국민연금의 적용의 사각지대 문제 및 짧은 연금가입기간의 문제를 개선하기 위한 지속적인 관심과 노력이 필요하며, 국민연금과 기초연금의 안정적인 관계 및 적정한 역할 분담이 필요함을 시사하고 있다. 또한, 그리스 연금개혁에서 나타나는 기득권 조정은 연금개혁이 지연되거나 왜곡되는 경우 현재 수급자들의 권리도 완전히 보호되지 않을 수 있다는 점에서 우리나라의 공무원연금 개혁에 중요한 시사점을 제공한다.;The purpose of this study is to examine the forced pension reform in Greece and draw its policy implications for the development of public pension systems in Korea. It explores the economic context and the political process of the pension reform in Greece, analyzes the contents of the Greek pension reform, and suggests its future directions. The results of the study are as follows: First, the 2008 global recession was the economic contexts for the 2010 Greek pension reform, and EU and IMF enforced the pension reform to reduce the national debt of Greece. The pension reform focused on the financial sustainability of the pension systems and the effects of the radical pension reform are profound. Second, the benefit level of the earnings-related pension was cut so significantly that there is a possibility of future poverty problems for the elderly. This study has two policy implications for the development of public pension systems in Korea. First, we need to reduce the coverage gap in the National Pension scheme and to extend the contribution periods in order to reduce the elderly poverty. Second, we need to balance the roles of the National Pension scheme and the Basic Pension and to develop the appropriate relationship between the two schemes.
한국에서 트랜스젠더의 의료접근성에 대한 논의는 전무하다. 본 연구는 한국 트랜스젠더의 의료접근성을 제도적 측면과 의료공급자 측면으로 나누어 살펴보고, 이와 관련한 외국의 현황과 그동안 진행된 논의를 소개하고자 한다. 트랜스젠더는 성별위화감 진단, 호르몬요법, 성전환수술 등의 성별 트랜지션(gender transition) 관련 의료적 조치와 더불어 성별정체성을 고려한 생애주기별 일반 건강관리 등과 같은 고유한 의료적 필요를 가지고 있다. 그러나 현재 한국에서는 트랜지션과 관련하여 의료적 조치에 대한 의료보장의 부재와 트랜스젠더에 대한 의료진의 편견 및 숙련된 의료진의 부족으로 인해 트랜스젠더의 의료접근성은 심각하게 제한되고 있다. 외국 연구들은 트랜지션 관련 의료보장의 부재는 호르몬 자가처방이나 자가거세 등을 증가시켜 건강을 해치며, 삶의 만족도를 떨어트린다는 점을 보여주고 있다. 이러한 결과에 기반하여 전 세계 118개국 중 43개국에서는 호르몬요법 또는 성전환수술을 의료보장에 포함하고 있으며 트랜스젠더 친화적인 숙련된 의료진을 양성하기 위해 의학교육과 의료진 수련 과정에서 다양한 시도가 모색되고 있다. 트랜스젠더에게 의료접근성은 신체적･정신적 질병을 치료하거나 예방하기 위해 필요한 것 이상의 의미를 지니는 중요한 삶의 조건이다. 한국 트랜스젠더의 의료접근성을 높이기 위한 적극적인 논의와 정책 수립이 필요한 시점이다.;This research examined current barriers that prevent transgender people from accessing health care in Korea and reviewed what has been discussed and what efforts have been made in some other countries to improve health care access of transgender people. Transgender-specific medical necessities include not only medical transition but also general health care throughout life course. However, Korean transgender people’s access to health care is significantly limited due to the absence of health care coverage on medical transition, cultural prejudice and discrimination of medical personnels, and shortage of well-trained surgeons. Previous literatures reported that lack of health care coverage on medical transition may increase self-prescribed hormone use and self-castration, which can severly damage their health. Currently, 43 out of 118 nations cover transition-related care in national health insurance or public health system. Studies also address that transgender-specific cultural competency educations and training programs are necessary for medical personnels. Future research is required to find effective strategies to improve transgender people’s access to health care in Korea.
